Reactivate an Individual Provider Record
Use the following steps to reactivate an Individual Provider.
1. Click Resources in the menu, then click Individual Providers.
2. The Individual Providers screen
displays. Click on the Provider you wish to reactivate.
3. The View Provider screen displays. Click Reactivate.
4. The Reactivate Provider screen displays. Click Continue.
5. The View Provider screen displays. The Status is updated to reflect that the Provider Record is now Active.
