Add a Service Provider to the Client Profile

Add an Individual Provider to a Client Profile


Use the following steps to add an Individual Provider to a Client Profile.

1.           Open the Client Profile.  Click Individual Providers to display the Individual Providers table. Click New.



2.           The Add Provider screen displays.


  1. In the Service Provider field, type or select the name of the Service Provider (this is optional).
  2. In the Provider field, type the individual provider name. The name will display outside the field, so make sure you select it to place it in the field.
  3. In the Service Hours field, enter the number of hours the provider is delivering to the participant, if applicable.

3.            Click Save.

4.           The provider's details are added to the Individual Providers table for the Client Profile.
